• Suitability Form #10068 is not required for:
  • Qualified Plans
  • TSA (403(b) cases that are FLOW premiums
  • ESI cases where the agent is a Registered Rep
    
• Applications do not need to include the Disclosure Summary "Is This Annuity Right for You? - Suitability form takes care of this.
• All LSW annuity cases (with the exception of ESI cases) need to go to Dallas including checks and transfer requests for new cases and after issue transfers.
• Dallas will be contacting the agents on any Suitability questions or problems, and may contact you on Transfer questions.
